In 1997 a group of surgeons founded The Surgical Clinic to develop patient-focused, conveniently located places that give patients access to surgical procedures close to home in Middle Tennessee.

Our goal is to involve you as much as possible in your treatment process. Through your providers, and convenient access to your reports, lab test results, and health information, we hope you will feel prepared and relaxed on the day of your procedure. OUR FOCUS IS ON YOU
Whether you are visiting an emergency room, have a hospital stay, or have general questions about our available services, surgical healthcare can bring up many questions. Through our Patient Portal you will have ongoing access, 24 hours a day and 7 days a week, to ensure you feel connected throughout your journey with us. We will set up a portal account for you, typically at your first clinic appointment. If you would like it setup in advance, please call your clinic to make a request. You will receive your login and password instructions within 24 hours via email.

Preparing For Surgery
NEW PATIENT FORMS
Our team has assembled a collection of the most commonly requested forms to help as you are preparing for surgery. These forms include new patient forms, as well as additional documents that are often needed by a patient’s school, employer, insurance company, or by supplemental insurance companies. It is very important that you find out from those entities whether any of these forms might be required to excuse your absence or apply for insurance, disability, or workers’ comp. We look forward to helping you and request that you print and complete new patient forms to bring to your first visit. You can either print the following forms as one packet, or each form individually.

COMPLETING NEW PATIENT FORMS
By clicking on the links above, you will be able to access any new patient forms you might need. There are two ways to fill out each form:
- Download the form(s) and print from your computer. From there, fill out the needed information and bring the form(s) with you to the office during your next scheduled visit.
- Most of these forms are enabled to allow you to type in your answers on your computer. To do this, download the form(s) to your computer. From there, open and fill out the form and save it to your computer once you are ready then send the filledout form(s) to the office via email or print the form(s) out and bring with you to your next scheduled appointment.
While copies of these forms are also available in your surgeon’s office, we urge you to fill out your part on each form you need before you see your doctor.

What does it mean to be a board-certified surgeon?

What is FACS?
FACS stands for Fellow of the American College of Surgeons. When you see FACS after a surgeon’s name, it means they are board-certified and have undergone additional peer review to confirm their ethics, skills, and professional excellence. Many of our surgeons at The Surgical Clinic proudly hold this distinction.
What is robotic surgery?
Robotic surgery allows doctors to perform complex surgeries with the precision and control only robot-guided movements can provide. It’s a form of minimally invasive surgery that enhances the capabilities of the surgeon. The surgeon sits at a console where they control the instrumentation of the robot’s movements. Robotic surgery results in very small incisions due to better magnification and control of incisions. These surgeries result in fewer risks and even faster recovery times.
